Our Adult Social Care & Health Ambition

We have aligned adult social care to the wider vision for health and wellbeing locally as described in the Council’s priorities.  Adult Social Care and Health (ASCH) contributes to achieving those priorities through its service plan.

We are committed to making our adult social care services easier to access, more personalised and effective.  We aim to help build personal, community and system resilience to make best use of our resources.  We want to support adults in the East Riding to live the life they want and do the things that are important to them as independently as possible living in their own homes.  We will work with partners to ensure that adults have care and support that is coordinated through joint working to meet their needs. We will be working closely with our partners in Vale and Coast Integrated Care System and this will strengthen joint services and promote better outcomes for local people.  You can read more about our partnership with health here.

The lasting impact of the pandemic means we will continue to respond and manage emerging pressures, prioritising those at the greatest risk.

 

 

 

 

We acknowledge that improving health and wellbeing depends on other local issues and we will consider how we connect to, and work alongside, economic growth and prosperity, learning and skills, housing and other priority areas.  We are committed to working with colleagues in children’s services so that we are able to plan for the needs of young adults as they prepare for adulthood.  This will include looking for services and arrangements as close to home as possible.

To promote healthy living, self-care and prevention we will look to release the capacity and innovation in our community.

We will seek out opportunities to work in partnership with other organisations, such as Humber and Coast ICS, NHS, voluntary and community, and private sector organisations.

Here in the East Riding of Yorkshire we support the principles of rights-based care that enables individuals to have some choice and control about how their needs are met. We are also committed to the Making it Real campaign and we will be using the series of focused ‘I’ statements to evaluate

 the success of our interventions. Enabling people to have normal lives, whatever their vulnerabilities should be at the heart of our service pathways ensuring they are accessible and that people are enabled to have a life, not a service. Demonstrating that services and support make a real difference to people is important to us.  

Through ongoing conversation with people using our services we will work towards the outcomes that they want to achieve in their lives.

 

 

We continue to work towards promoting wellbeing and reducing dependency.  The council has responsibilities for preventing, reducing and delaying health and social care needs developing.  This includes people who do not have any current needs for care and support, those with emerging needs, and carers.  The East Riding of Yorkshire’s  ‘Your Life, Your Way’ change programme is driving improvements designed to prevent, reduce or delay the development of care needs.
